Filing an Objection to the Receiver's Recommendation
On April 7th, 2006, Receiver Warfield filed with the court
a Claims Report which contained his recommendation to the court
regarding how each claim should be addressed. If you have reviewed the
Claim Report and you wish to submit an objection please follow the
instructions below.
If you have any objections to the report you must submit a letter of
objection to:

Must explain the exact item of dispute

Must be POSTMARKED by May 15, 2006

MUST NOT be sent to the court.

followed, then it is likely that the amount will remain the same. If you have

The following documents are related to the claims process:
Regarding Investors/Annuitants who have died.
If an investor or annuitant in Mid-America Foundation has died, the
proof of claim should be signed by the appropriate representative/executor of
the deceased investor's estate.
If there is no representative/executor of the estate, one of the
heirs should sign the proof of claim and attach a copy of the deceased
investor's will, if one exists, an explanation of the reason that there is no
representative/executor of the estate, and the names and addresses of all of
the deceased investor's heirs.
If there is no representative/executor, you should also consult with
an attorney specializing in probate matters.
