Documents related to claims...

The following documents are related to the claims process:

Regarding Investors/Annuitants who have died.

If an investor or annuitant in Mid-America Foundation has died, the proof of claim should be signed by the appropriate representative/executor of the deceased investor's estate.

If there is no representative/executor of the estate, one of the heirs should sign the proof of claim and attach a copy of the deceased investor's will, if one exists, an explanation of the reason that there is no representative/executor of the estate, and the names and addresses of all of the deceased investor's heirs.

If there is no representative/executor, you should also consult with an attorney specializing in probate matters.
